Fire agencies run incidents, inspections, pre-plans, hydrants, and personnel across systems that were never built to talk to each other. The pre-plan for the building on fire lives in a binder on a rig that is not the one that arrived first, so command opens to a structure it cannot see inside. Hydrant flow data and inspection findings age in a spreadsheet no one trusts. Apparatus and hose-test records sit in another binder until the day they are needed for an audit. There is no single operational picture, so command works from memory and radio traffic while the NERIS report gets rebuilt from scratch days later.
