NEMSIS validation in most systems happens at submission time, so the error surfaces after the encounter is closed and the chart has to be reopened and re-signed. Charts finalize without the structured fields the biller needs — the transport reason, the level of service, the medical-necessity narrative — so the claim stalls in a clarification queue. Every reopened chart is a chain-of-custody question waiting to be asked.
