Manual crew paging for EMS transport is a phone tree run under time pressure by whoever is free to dial. The dispatcher who knows who is certified for the call is not always the one making the page, so a critical-care leg gets offered to a crew that cannot legally run it. Units sit idle while the right medic is two names down the list and nobody is sure who already said no. When no one answers, escalation is another manual round of calls instead of an automatic next step — and the minutes lost are minutes the patient is waiting and the truck is unbillable.
